SBA Forms
1) SBA Form 912 - Complete for all individuals owning stock in the company.
2) Business Debt Schedule - This must be dated the same date as the Interim Balance Sheet and showing all liabilities of the company.
3) Aging of Accounts Receivables and Payables - This report must be dated the same date as the Interim Balance Sheet
4) IRS Form 4506-Request for Copy or Transcript of Tax Form - Each stockholder must submit a signed copy of this form
Commercial Loan Forms
1) Applicant's Information Sheet
2) Management Resume(s) - Complete for all individuals owning more than 20% of the company's stock.
3) Personal Financial Statement(s) - Complete the form for all stockholders owning more than 20% of the company's stock
4) Business History/Plan - in addition to form please include any brochures, advertising material or printed history of the business.
5) Projected Operating Statement - Include assumptions for projections
